Is it possible to merge ROM.BIN, IMAGE.BIN and BACKUP.BIN into a single BIN file? If so, at what offsets do each go, and what should the resulting file name be?

You can certainly merge rom.bin and image.bin - see http://www.digi.com/support/forum/viewthread_thread,2269 for a link to a utility to do it
Yes, I’ve been doing that for a while (using my own utility). I was wondering if backup.bin could be added so that everything can be done in a single update.
